Briggs Wound/Skin Healing Record provides for pertinent documentation of wound and/or skin injuries including Arterial, Diabetic and Venous Ulcers, DTI, Pressure Injury and other such injuries.
The form includes:
| • | Anatomical diagrams to identify the location of wound/injury |
| • | Identification of the date healed as well as the date Physician was notified of wound status, culture and results, etc. |
| • | Recording size, depth, stage, exudate, odor, wound bed, surrounding skin and wound edges, tunneling, undermining, etc. |
| • | Identification of pain including location, intensity, and frequency (Wong-Baker FACES scale) |
| • | Response to treatment, updating of care plans, and comments |
| • | Four separate evaluations of the same wound on one form |
