Use this form to request instructions from the attending physician regarding dose reduction attempts.

CMS regulation, §483.45(e)(2), requires residents who use psychotropic medications receive gradual dose reductions (GDRs), unless clinically contraindicated, in an effort to discontinue these drugs. For any resident who is receiving a psychotropic medication, the facility must show evidence that a GDR has been attempted unless clinically contraindicated. If there is no evidence of a GDR and there is no description of the clinical contraindications, then facility noncompliance exists.

Per the Centers for Medicare & Medicaid Services (CMS), the purpose of the required GDR or tapering of medication is to find an optimal dose or to determine whether continued use of the medication is benefiting the resident or could have dangerous side effects.
