Physician Orders NCR

Briggs Form 1264 allows for three series of orders. Duplicate copies are spot-glued at the left between perforations, allowing them to be removed from the form. as a set and later separated for distribution. A botton stub holds all parts in place. Briggs Form 1265 is uniquely designed to allow you to remove copies without taking the actual form out of the patients chart. After the physician writes an order, Parts 2 and 3 are removed as a set, then separated for distribution to Nursing and Pharmacy. Pharmacy and Nursing copies are cut below the stub allowing quick removal while the top sheet stays in the chart. A backing sheet (Part 4) holds all parts together. Available with 5-hole top punched (1265P) or non-punched (1265).
