The Mood Collection Tool includes both the Resident Interview - PHQ-2 to 9 - and the Staff Assessment when the resident is unable to complete the interview.
If the resident is not able complete the Mood interview, the clinician marks that item on the reverse side then continues with the staff assessment. If the resident completed the Mood interview, that item is checked and the staff assessment is not completed.
