Interdisciplinary Post-Fall Assessment

The Interdisciplinary Post-Fall Assessment form is used by the interdisciplinary team to assess a resident after a fall has occurred.
This assessment looks at all elements that may have contributed to the fall. Review of assessment data gathered assists the IDT in developing a resident-specific care plan to prevent further falls, reduce injury, etc.
The form is considered part of the facility's Quality Assessment and Assurance Process and thus is considered a confidential QAA Committee record
