F661: Recapitulation of the resident’s stay that includes, but is not limited to:

| • | Reconciliation of all discharge medications, both prescribed and OTC |
| • | A post-discharge plan of care that is developed with the participation of the resident that will assist in adjusting to his/her new living environment. This post-discharge plan of care must indicate: |
• Where the resident plans to reside | |
• Any arrangements made for follow-up care and | |
• Any post-discharge medical and non-medical services |
