The Drug Regimen Review - MDS/SNF QRP provides for documentation of the Drug Regimen Reviews that are required/conducted when a resident is admitted for skilled Part A services as well as throughout the skilled stay. The form also references F756 which requires the drug regimen of each resident to be reviewed at least once/month by a licensed pharmacist.
In addition to the date and time of contacts as well as resident, pharmacist, and physician/physician designee identification, the form includes:
