Physician Action Report—Drug Regimen Review provides a record of communication between the Pharmacist and the Physician regarding concerns following a drug regimen review.

This form provides for:
• Identification of clinically significant issues including drug, dose, frequency, route
• Detailed description of the Pharmacist's findings, concerns, and recommendation(s)
• Physician comments and orders

A copy with the Pharmacist's report and corresponding documentation is kept on the resident's record with a copy mailed to the Physician. Upon receipt of the completed review by the Physician, that final copy is retained on the resident's record.
