INTERACT® Stop and Watch Early Warning Tool

The INTERACT® Stop and Watch Early Warning Tool can assist in early identification and communication of a change in condition. STOP AND WATCH is an acronym which arranged vertically provides a list of condition changes that staff can observe for and document by circling the letter and communicating the possible change to nursing staff. When an acute change in condition occurs, the Stop and Watch Tool can be used by CNAs to identify changes in residents and clearly communicate those changes to the licensed nursing staff. This tool can also be used by staff who have direct contact with residents and are in a position to observe changes, including rehabilitation therapists, environmental services, and dietary staff. It can also be used to educate families so that they can report changes in condition they notice while visiting.
