Health History Summary (MNRS)
Early intervention measures can help reduce risk and ensure positive pregnancy outcomes. The physician will be able to quickly make an assessment of the risk status of the patient based on the patients general health history as well as menstrual, pregnancy, and genetic histories.
This comprehensive history is completed on the first visit. It may be completed by the patient or by the physician during the interview and exam.
