Social Service Evaluation
Briggs Social Service Evaluation form provides for evaluation and care planning of the resident's overall status from the perspective of social services. Areas addressed include:

•Sensory Concerns (Vision/Hearing/Communication)
•Family Relationships/Support Systems
•Psychosocial Well-Being/Socialization (now includes admissions and continued stay)
•Cognitive/Mental Status (now includes admissions and continued stay)
•Mood State/Behavior Problems/Psychosocial Adjustment (now includes trauma in addition to other psychosocial considerations)
•Personal Needs/Pay Status
•Referrals and Outside Resources
Co Each category contains a prompt for referrals indicated and the location where additional information can be found in the resident's medical record.
