Understanding Resident Rights is designed to provide residents/resident representatives/families with the laws
guaranteeing their rights while living in a long-term care facility. Those rights are identified by category and are
easy to locate using the Table of Contents.

The Guide incorporates the Resident Rights from Appendix PP of the State Operations Manual, the Privacy Act
Statement and Important Contact Information all in one resource - a resource that the resident and/or family will
keep and reference as questions arise. Also included:

Facility/Staff Information - completed by the facility within the booklet - provides the facility name, address, and phone as well as the names of the Administrator, Director of Nursing, and other department heads. This provides identification of key staff members for the resident/family.
Important Contact Information required by law - facility completes those pages within the booklet, i.e., State Survey Agency, State Ombudsman, LCA, Medicaid Fraud Control, Aging, and Disability Resource Center, etc.
Questions About My Rights - resident questions can be written down as they arise so the questions can be answered by staff:
• Next Appointments
• Concerns/Questions to Talk About with My Doctor
• Personal Notes to My Doctor/Family/Caregivers
• Medication Profile
• Location of Important Documents with Your Wishes
• My Contact Numbers
• Acknowledgment Signatures - perforated for easy removal following review and signature;
readily accessible document for retaining in the facility's Business Office file.
