Updates include:
  • Updated Vital Signs area provides for the collection of data each shift for 1st 48 hours, helping to establish a baseline for temperature, pulses, respiration, and blood pressure. This area now includes current weight and scale used as well as current height and PERRLA
  • Additions to Condition on Admission such as presence of dentures, indwelling catheter, speech, etc.
  • NEW Pain section includes Wong-Baker FACES Pain Rating Scale plus other information regarding patient/resident pain
  • NEW reverse side provides for specific identification and documentation of skin status
  • Also on the NEW reverse side, there is a section for additional Comments/Notes by the clinician
  • NEW Signature/Date section on the reverse side provides for the identification of two (2) nurses completing the admission record
