Incident Report QA/CQI Log
Use Briggs Form 3090 to log incidents/accidents on a monthly basis for continuous quality improvement. Updated to include:
- A checkbox for Other incidents that do not fit into the defined items available
- Shift categories of 1st, 2nd and 3rd shifts to better accommodate two 12-hour shifts or other shift designations
