Record all in-service and educational training sessions for staff, volunteers, etc. with this newly revised form. Key features include:
• | Easy identification of employee group(s) present for training (Administration, Nursing, Dietary, etc.) |
• | Quick identification of training categories using checklist format. Includes the fifteen (15) training items required by CMS. |
• | Section for a summary of the training session with a prompt to attach a copy of handouts or other learning materials used |
• | “Conducted by:” collects name, title, checklist and qualification(s) of presenters |
• | Signature/title of the individual completing the in-service training report |
• | The reverse side provides for attendee signature/title collection |
