EZ Pathways Axial Visit Sheet - CAD

EZ PathwaysTM is comprised of a family of 30 disease-state specific clinical pathways. While one booklet typically covers an episode of care, axial sheets are available to use when additional visit are needed. Similar to a Visit Note in a booklet, an Axial Sheet is designed to:
• Define the specific patient goals.
• Identify the health status of a patient at any point in time.
• Incorporate two, three or more co-morbidity interactions on a single page.
• Identify standards of practice at each visit.
• Numerically quantify areas of improvement, stabilization or decline.
• Provide verification of required teaching and skill building.
• Provide measured evidence of progress toward goals.
• Acts as a nurse’s note and outcome measures all on one page.
